Dear Parent/Guardian,
Under New York State regulations, all new students and students entering Pre-Kindergarten and/or Kindergarten, and grades 1, 3, 5, and 7 are required to have a medical examination for the new school year. Medical examinations should be completed by your own physician whenever possible. The family physician is the most familiar with the health history of your child and is therefore best able to evaluate his/her needs. If this is not possible, the school physician will be at the school in December. All documents must be received by and approved by the St. Martin of Tours Health Office within two weeks of the start of school. Failure to do so will result in the student being excluded from school in accordance with New York State Law and the Diocese of Rockville Centre.
